This is the official form for requesting tournament approval.

This form is required for approval of your tournament – including if it involves clubs from outside USGAA’s jurisdiction (eg. Canada, New York, Ireland etc.)

Please complete all of the fields to the best of your ability. Those marked with an asterisk (*) are required.

  • If there is an Event sponsor(s) or commercial partner secured for this event – or if you are currently working to secure a sponsor – please list them here. Note that the USGAA must approve non-local sponsors for any USGAA events. This includes companies or organizations that operate at a National (USA) or International level.
  • If the event is being financed by other funding such as grants, donations, etc, please estimate how much such contributions are likely to be.
  • Participating Teams

    It is important to list the clubs/teams participating - particularly any that are outside of the host division or outside of the USGAA jurisdiction - e.g. Canadian clubs, unaffiliated clubs, etc. Note that under Official Guide 6.40(c), any participating teams from outside USGAA will need approval from their own Controlling Authority eg. County Board, National Board etc.
